What you will do:

About You:
* Previous experience working within an Integrated Hospital Discharge Team is essential
* Familiarity with the Eclipse system would be advantageous
* Demonstrated ability to operate efficiently in a dynamic and high-pressure environment
* A full UK driving licence and access to a vehicle is required
* Strong interpersonal and communication skills to engage with diverse populations
* A degree in Social Work or related discipline, with a commitment to continuous professional development
