A vacancy has arisen within our friendly, diverse Early Intervention Team for a full‑time care coordinator.
The post is based in Swinton with an expectation for city‑wide travel in order to visit people in their own homes or within community venues.
Early Intervention Services aim to;
Reduce the stigma associated with psychosis.
Improve professional and lay awareness of the symptoms of psychosis and the need for early assessment and intervention.
Reduce the length of time young people remain undiagnosed and untreated.
Develop meaningful engagement, provide evidence‑based interventions and promote recovery during the early stage of illness.
Staff training that ensures that all staff are equipped with the knowledge and skills to provide the NICE concordant packages of care and Best Practice recommendations.
Increase stability in the lives of service users, facilitate development and personal fulfilment.
Provide a user‑led service, i.e. a seamless service available for those from age 14 to 65 years (on acceptance) that effectively integrates child, adolescent and adult mental health services and works in partnership with primary care, education, social services, youth and other services.
Ensure that care is transferred thoughtfully and effectively at the end of involvement.
You will be responsible for
* with the support of an MDT
* a caseload of people experiencing a First Episode of Psychosis.
You will deliver a NICE-concordant package of care for each person, ensuring that they have access to evidence-based recovery interventions whilst maintaining adherence to the Trust and national performance requirements for Early Intervention services.
You will complete initial assessments and participate in the duty and back‑up duty rotas, attend and contribute to team meetings, and have access to regular training and development to help you thrive in your role.
