Job Summary

The focus of the role will be to refer and signpost to the correct services to support early intervention and prevention of reliance on more specialist services, while reducing pressure on GP practices. Referrals will be received from primary care health and social care professionals aligned to the practices, as well as external organisations and patient self-referrals. Individuals can be referred to Community Link.


Main Duties of the Job

Workers will support a range of issues including social isolation, housing instability, debt/financial hardship, education/employment matters, low-level mental health needs, and addiction/substance misuse/smoking. They will help patients navigate these challenges and connect them with appropriate services.

A full UK driver’s licence and a vehicle will be required as you will be expected to travel across several different bases.

Job Details

Pay scheme: £27,485 to £30,162 a year pa/pr. Contract: fixed term. Duration: 11 months. Working pattern: part‑time. Reference number: 810‑7534161. Location: 358 Marfleet Lane, Hull, HU9 5AD.


Person Specification

Essential

* Level 3 in Health and Social Care (or working towards an equivalent qualification) or demonstrable equivalent experience
* Completion of Social Prescribing e‑learning programme or willingness to complete this as part of induction
* Significant understanding of the role of social prescribing within the wider health and social care sector
* Understanding of voluntary and community sector organisations and their contributions to health and wellbeing
* Knowledge of local communities, resources, and assets
* Knowledge of factors influencing behaviour and behaviour change approaches
* Understanding of health and social care infrastructure and service provision to local populations
* Understanding of factors influencing individual health and wellbeing
* Understanding of community development and asset‑based approaches
* Understanding of barriers and challenges people face in accessing local activities and support
* Understanding of accurate record keeping and information governance requirements
* Knowledge of Microsoft Office (Outlook, Excel, Word, PowerPoint)
* Knowledge of primary care IT systems such as System 1 and EMIS
* Knowledge of health and safety requirements for risk assessment of venues and home visits
* Demonstrable experience working with community groups, voluntary organisations or a GP practice
* Experience of working with individuals on a one‑to‑one basis
* Experience of building relationships and partnerships
* Experience of working collaboratively with health care and non‑health care professionals to meet patient needs
* Experience of peer support and/or mentoring others

Desirable

* Diploma in a relevant subject area
* Clait, ECDL or equivalent IT qualification
* Training in motivational coaching and interviewing or equivalent experience
* Experience of delivering presentations
* Experience of collating and presenting information
* Experience of involvement in service monitoring and evaluation
