Epsom and St Helier University Hospitals NHS Trust provides a range of medical services to approximately 490,000 people living across southwest London and northeast Surrey. In addition, we provide more specialist services, in particular renal and neonatal intensive care, to a wider area, covering parts of Sussex and Hampshire.
We serve an area that is rich in diversity, with a mix of urban and rural areas, and differing levels of quality of life. We cover some of the most prosperous postcodes in the country, as well as some poorer areas. The SWLEOC is an NHS Treatment Centre providing regional elective orthopaedic surgery services (including inpatient,day-case and outpatient).Established by the four South West London acute Trusts to deliver strategic change in thedelivery of planned orthopaedic care, the SWLEOC provides high quality, cost efficient, elective orthopaedic servicesrankedamongstthebestintheworld.

Detailed Job Description And Main Responsibilities
Toprovideacomprehensivepharmaceutical service,workingtowardsexpertlevelknowledge.
TofurtherdevelopmanagerialskillsandexpertisebyworkingtoobjectivesagreedwiththePrincipalPharmacistSWLEOCandthe DeputyChiefPharmacist.
To liaise closely with the Principal Pharmacist SWLEOC to contribute to the maintenanceand development of evidence based treatment guidelines/shared care protocols/patientgroupdirections(PGDs),whereappropriate,forthedivisionandcontributetoPharmacy/Trustpolicies as required.
To work in liaison with the pharmacy leads to ensure changes to practice meet requiredstandardsand are appliedTrust-wide.
TodeputiseforthePrincipalPharmacistSWLEOCasnecessary.
To monitor the pharmacy service to the SWLEOC, working with the pharmacy leads toinitiateimprovements orchangesas required.
Appropriatelyescalateissuesthatmayimpactthedeliveryofroutinepharmacyservicesdeliveredto SWLEOC.
Toreportandinvestigateerrors,complaintsandincidentsinlinewithTrustanddepartmentalprocedure.
To facilitatethe sharing oflearningfromerrors, complaintsandincidents
Toundertakeriskassessmentsandmanageandreportriskasrequired.
Toattend/participate/presentindepartmentalClinicalGovernancemeetingsasappropriate.
Toproducerotasfordispensary,wardcoverandlockingup.
Toundertakedispensing/checkingofprescriptionsforpatientsusingthePharmacyandTrustComputerSystemsandinaccordancewith departmentalpoliciesandprocedures

To participate in the daily Consultant led Multi-Disciplinary Team (MDT) ward rounds on theSWLEOCwarddepartments,providingexpertpharmaceuticaladvice.
Toundertakemedicinesreconciliationforeachpatientonadmissiontoensureprescribedmedicationsareappropriateas earlyas possibleintheirstay.
To pharmaceutically screen medication drug charts and prescriptions to a safe and legalstandardtherebyensuringtheappropriate,cost-effective,andtimelysupplyofmedicines.
In a timely manner, pharmaceutically review and validate the medicines reconciliationscompletedby ClinicalPharmacyTechniciansandactonallrelevantpharmacyhandovers.
