Overview
The aim of Adult Community Services is to provide a first-class healthcare service across in-patient and community services for a broad multicultural community in Hackney and the City. We have an excellent training, development and support mechanism in place to enable staff to meet high healthcare standards.
We have Deputy District Nurse Team Leaders vacancy within the Adult Community Services Department to provide nursing care to allocated patients within their homes.
Responsibilities
* The post holder is responsible for the planning, implementation and evaluation of nursing care needs and the delivery of high quality, up-to-date care to allocated patients in their homes and/or a variety of settings.
* Work within a team for a defined number of patients and operate under the supervision of the Clinical Operations Manager.
* Be responsible and accountable for the management of a caseload in the absence of the Clinical Operations Manager/District Nurse.
* Collaborate with aligned GPs to your team and act as advocate and key channel of communication for patient care.
