City Health Partnership Ltd (CHPL) is seeking a highlymotivated and enthusiastic Senior Practice Nurse with a V300 IndependentPrescribing qualification to join our team. The role involves the clinicalassessment and management of patients primarily within a general practicesetting, focusing on long-term condition management, including asthma,diabetes, COPD, and hypertension.

Consultations will be conducted face-to-face, by telephone,or via digital platforms, managing undifferentiated illness and acutepresentations related to long-term conditions. The successful candidate willhave a strong background in general practice nursing, with a thoroughunderstanding of QOF (Quality and Outcomes Framework) requirements,immunisation programmes, and current health campaigns.

The Senior Practice Nurse will ensure the highest standardsof clinical care across CHPL, working within their professional boundaries andsupporting the implementation of organisational policies and procedures. Theywill work closely with the Clinical Team Leader to support and improve serviceprogression, contribute to service improvement initiatives, and mentor juniornursing staff. This role demands advanced clinical skills, includingindependent prescribing, to provide comprehensive, holistic patient care.

The service opening hours are Monday-Friday 8am-6.30pm andit is expected contracted hours will be worked during these times.


Main duties of the job

The post holder is accountable for the delivery of safe, high-quality, and effective clinical care within primary caremedical services. They will provide leadership and support to all nursing staff, collaborating closely with otherleaders and the multidisciplinary team to achieve service priorities. Clinically, the role is focused on implementingevidence-based practice to meet the needs of a diverse patient population, ensuring optimal health outcomes.

Person Specification


Knowledge

* Indepth knowledge of primary care
* Proficient in SyStmOne, Ardens Templates, andcomputer use
* including Microsoft Word, Outlook and Excel
* Specialist knowledge of personnel
* Full understanding of relevant policy drivers, Quality Outcome
* Framework and contractual obligations including Local enhanced Services in primary care settings
* Full understanding/application of relevant clinical practice/standards/audit within identified clinical area
* Knowledge of project management systems


Experience

* Evidence of continuing professional development that is Interview
* relevant to the sphere of care
* Significant experience in a leadership role in Primary Care
* Proven ability to formulate and implement strategic plans
* Demonstrate competency in a wide range of skills including phlebotomy, wound care, health promotion, foreign travel
* health advice, smears, patient health checks, sexual health,
* family planning and contraception.
* Caseload management skills including planning, treatment and
* Experience of managing patients with chronic disease or long term conditions
* Able to demonstrate development in practice and expertise
* Professional understanding of the treatment and care of patients in primary care settings and all related national
* guidance
* Experience of training health care workers and developing training plans
* Evidence of specialisation in a clinical area relevant to the post
* Understanding and ability to identify and support both service
* and cost improvement programmes.
* Experience in chairing meetings
* Extensive experience of clinical operations and delivery in a primary care setting.
* A proven track record of involvement with projects


Qualifications

* Registered General Nurse
* Degree or Advanced Diploma in nursing studies
* Postgraduate qualification in relevant nursing studies or
* equivalent level of knowledge, skills and experience
* Non-Medical prescribing qualification (V300) or working
* Mentor/Teaching Qualification
* Diplomas or equivalent qualification or certificate in the care &
* treatment of patients with chronic disease in two or more of
* the following: diabetes, respiratory medicine, coronary heart
* disease.
