We have an exciting opportunity for a Band 3 clinical support worker to join a team of experienced children's nurses delivering nursing care, advice and support to children and young people 0-19, with complex health needs and long-term conditions, within their home and other care environments.

The role involves supporting the delivery of high-quality care for children and young people within the community setting under the direction and supervision of a registered nurse. In addition, providing practical care, assisting with clinical tasks, and supporting families in managing their child's health needs at home.

This is a part-time position, 22.5 hours per week, for a service which provides care Monday-Friday 8-6. The week will consist of three 7.5 hour shifts of 09:00-17:00. We based at Snapdragons Children's Centre in Strood.

Key Responsibilities

Assist community children nurses (CCN) with the delivery of planned care for children and young people, including those with complex health needs.

Carry out agreed and delegated clinical tasks (such as observations, basic wound care, equipment checks and basic assessments ) within defined competencies.

Support families and carers in understanding and managing the child's care plan, signposting to further advice when appropriate.

Record interventions delivered accurately and promptly in line with service policies and professional guidance.

Escalate any concerns about the child's health or wellbeing to the supervising CCN without delay.

Maintain infection prevention and control standards in all settings.

Contribute to safeguarding by recognising and reporting any concerns regarding the welfare of a child.

Participate in team meetings, supervision, and training to develop and maintain skills and knowledge.

Maintain confidentiality and adhere to data protection requirements at all times.

1. Communication and relationship skills

Deliver effective communication within the children's nursing team, the child and family and other professional main stream services.

Communicate complex patient information, verbally and in writing to other professionals.

Provide advice, motivation and education to patient's family and carers in respect of their medical/health condition, where there may be barriers to understanding, being empathetic and providing reassurance.

1. Knowledge, training and experience

NVQ level 3 or equivalent preferred.

Have an understanding of healthcare in the community setting and functioning of an integrated working approach to care.

1. Analytical and judgement skills

Have the ability to prioritise and respond appropriately to patient referrals.

Be proactive in the management of risks.

Have the ability to recognise when to seek advice and feedback from trained staff when there are changes in a child's condition or needs

1. Planning and organisational skills

Have effective time management skills to meet the demands of allocated patient care.

Support the community children's nursing team in the management of the caseload.

Have the ability to adapt and be flexible in responding to changing needs and demands of the service.

1. Physical skills

Undertake a range of clinical skills including blood pressure measurement, urinalysis, weighing and measuring, wound care, and other nurse delegated tasks with basic competency levels signed off by registered nurses.

Basic IT and keyboard skills, including the use of Word & Excel, are required

1. Responsibility for patient care

To liaise with other services to provide a seamless patient experience and quality outcome

Actively promote equality and value diversity

Provide a range of care interventions to children and young people in the community or clinic settings, working alone or with other team members as per the child's health care plan to ensure that individual needs are met.

Maintain clinical competency and an awareness of health and social care to ensure high standards of care.

1. Responsibility for policy and service development implementation

Maintain a professional manner and act as a role model, adherence to organisational policies and protocols.

Actively participate in clinical audit and clinical governance activities including risk assessments.

Participate in data collection, activity reporting and outcome measuring for community children's nursing.

Actively progress integrated/interprofessional patient care and collaborative working.

Contribute to the achievement of organisation and service objectives.

To be aware of safeguarding vulnerable adult/children issues, being patient advocate and reporting concerns to senior members of the team.

Be aware of legal and ethical responsibilities to protect patients' confidentiality.

1. Responsibilities for financial and physical resources.

Educate children & families to ensure awareness of the correct use of equipment and consumables.

Promote compliance to the organisation's procedures for supply of consumable.

Assist in maintenance of stock levels for the team and patients.

1. Responsibilities for human resources (HR)

Participate in own appraisal and clinical supervision.

Take responsibility to maintain personal clinical competence.

1. Responsibilities for information resources

Ensure records are kept up to date and safeguard patient confidentiality by being aware of professional, legal and ethical responsibilities.

Record and submit accurate, timely data and information to inform caseload management and service development.
