We are currently seeking an experienced Band 7 Mental Health Practitioner (MHP) to join the Mental Health Integrated Community Service (MHICS) in Leatherhead on a part-time, ongoing agency basis. This is an excellent opportunity to work within a dynamic team delivering person-centred care in a primary care setting.
This role is suitable for Registered Mental Health Nurses (RMN), Occupational Therapists (OT), or Social Workers (SW) with relevant NHS experience and a solid understanding of community mental health pathways. Key Responsibilities
Conduct comprehensive mental health assessments and collaboratively develop care plans with service users.
Work closely with GPs and other primary care staff to provide integrated mental health support.
Monitor mental state, manage clinical risks, and support treatment adherence, including medication monitoring where appropriate.
Provide short-term interventions and facilitate referrals or signposting to relevant services.
Maintain accurate and timely clinical records using SystemOne and EMIS.Essential Requirements
Current professional registration with NMC, HCPC, or Social Work England.
Recent NHS experience in a Band 6 or Band 7 mental health role.
Proficiency in using SystemOne and EMIS systems.
Solid understanding of primary care and integrated community mental health services.
Strong communication, assessment, and risk management skills.Benefits
