We are excited to be offering an exciting opportunity for a Highly Specialist Pharmacist in Paediatric Palliative Care and Academic Link role. This unique role offers the successful candidate a combined position, working three days per week within the Evelina London Pharmacy team and two days per week with the academic pharmacy team at King's College London university.

Provision of a clinical pharmacy service to the Evelina London Children's Hospital Palliative care team

Supporting the ongoing development and collaboration of the undergraduate MPharm curriculum and in placement provision to ensure teaching reflects current clinical practice

Provision of academic teaching and professional role modelling to undergraduate and postgraduate pharmacy students at KCL, specifically the postgraduate Pharmacist Independent Prescribing (PIP) PG Cert

Support in the provision of post-graduate academic programmes across GSTT such as the UCL MSc in International Practice and Policy

Support in the provision of the pharmacy departmental academic offerings such as the Visiting Pharmacy Professional (VPP) programme and International Foundation Masterclass in Paediatric Pharmaceutical Care.

Support the Principal Pharmacist for Education and Workforce Development with education, training and development across GSTT and work collaboratively with other members of the Pharmacy education and training team

When required, participate in the provision of pharmaceutical care at ward / clinic level.

Leadership

To lead clinical pharmacy services for paediatric palliative care patients in accordance with the priorities set by the post holder and the clinical lead pharmacist, in order to ensure safe, clinically effective and cost efficient use of drugs in these patients.

To ensure the delivery of pharmaceutical services to paediatric palliative care patients

To work collaboratively with senior pharmacy, medical and nursing staff, the multidisciplinary team and management and to participate in the pharmacy services agenda in accordance with the agreed objectives

To act as a Professional Tutor for identified clinical pharmacy staff

Management of service

To provide clinical pharmacy services for paediatric palliative care patients in accordance with the priorities set by the post holder and clinical lead pharmacist in order to ensure safe, clinically effective and cost efficient use of drugs in these patients.

To identify and prioritise clinical pharmacy services to the paediatric palliative care patients within the directorate

To ensure national and local agendas are delivered

To follow and conform to relevant standards of care

To coordinate and support the development and maintenance of the paediatric formulary

To identify and manage risks in relation to medicines use according to policy/protocol

To establish processes for the setting and implementation of standards of care

To identify and manage changes that needs to occur to achieve new standards or care and to identify and manage the risks involved

To identify resources required to improve clinical pharmacy services

To delegate authority appropriately to junior staff and to evaluate their performance against identified criteria using Trust tools

To provide evaluated pharmaceutical and financial information for paediatric palliative care patients

To identify new prescribing practices that may impact on budget (horizon scanning)

To demonstrate professional accountability to service users, stakeholders and the profession

Evaluation of Service

To monitor clinical pharmacy services for paediatric palliative care patients in accordance with the priorities set by the post holder and clinical lead pharmacist in order to ensure safe, clinically effective and cost efficient use of drugs in these patients.

To evaluate the contributions of the pharmacists to the directorate

To ensure that the quality of the paediatric palliative care service is recognised as a leader in the national field, in line with the corporate objectives

To ensure departmental standards are met

To undertake clinical audit as part of the multi-disciplinary audit process

To undertake assessments / evaluations as required

Clinical Practice

To act as a clinical role model for paediatric pharmacists and demonstrate the ability to provide safe, clinically effective and cost-efficient use of drugs to paediatric palliative care patients.

To demonstrate expert pharmaceutical knowledge within paediatric palliative care

To manage and make appropriate referrals

To follow legal, ethical, professional and employers' codes of conduct

To plan, manage, monitor and review therapeutic programmes for paediatric patients

To enhance the quality of patient care

To manage difficult and ambiguous problems

To tolerate uncertainty

To demonstrate an advanced level of clinical reasoning and judgement

To demonstrate a whole-system patient-focused approach

To demonstrate professional accountability

To identify trends in drug errors from Radar reports and pharmaceutical misadventure data and develop strategies to minimise these

To demonstrate awareness and commitment to the Trust's Clinical Governance Agenda

Research and Development

To develop clinical pharmacy services for paediatric palliative care patients in accordance with the objectives set by the post holder and clinical lead pharmacist in order to advance the safe, clinically effective and cost-efficient use of drugs in these patients.

To demonstrate a critical approach towards pharmacy practice

To develop, implement, monitor and evaluate protocols and guidelines

To develop clinical pathways

To integrate research evidence into practice

To identify gaps in the evidence base

To actively seek to improve the ward and clinical services provided

To develop a research strategy and undertake and publish practice research

To guide and support others undertaking research

To liaise with clinical staff in order to assist with the identification and supervision of undergraduate and postgraduate projects

To assist in the development of pharmacy practice research links between KCL and the hospital

To promote the publication and presentation of research undertaken at both the hospital and university

4.6 Education and Training

Academic Responsibilities

* To lead on the design and delivery and assessment of the postgraduate Pharmacist Independent Prescribing (PIP) PG Cert
* To carry out administrative duties associated with the PIP PG Cert programme, such as scheduling of teaching and assessments
* Act as a Personal Tutor for allocated KCL undergraduate and/or postgraduate students
* To act as a professional role model to undergraduate and postgraduate students
* Contribute to the development and progression of pharmacy education and clinical placement links between KCL and GSTT
* Contribute to and develop the MPharm programme in line with current practice
* Design, deliver and assess elements of the PIP PG Cert including the Objective Structured Clinical Examinations (OSCEs)
* Participate in clinical pharmacy teaching and multi-disciplinary teaching as appropriate
* Co-ordinate GSTT input into the MPharm and postgraduate programme and manage the relationships with clinical teaching staff to facilitate teaching and assessment
* Work with pharmacy staff hosting placement students to develop their skills in education, identifying the training needs of pharmacy staff to undertake the roles required
* To coordinate technical logistics for students undertaking placements at GSTT
* To maintain documentation
* To design and supervise undergraduate research projects as appropriate
* To contribute to student selection for admission to both MPharm and postgraduate professional programmes
* Assist in KCL academic administrative duties such as examinations, curriculum development, professionalism and fitness to practise investigations, and pastoral care
* Maintain a portfolio of practice
* Participate in education and training programmes to develop skills as part of a commitment to continuing education and the concept of lifelong learning

Trust based E&T Responsibilities

* To support the Trust Education and Training team working for the Evelina team and the across all sites at GSTT.
* To identify own training needs
* To participate in education and training programmes to develop skills as part of a commitment to continuing education and the concept of lifelong learning
* To maintain a portfolio of practice
* To participate in the clinical training of pharmacy staff, ward pharmacists and the multi-disciplinary team as appropriate

Other duties

To undertake clinical duties over the weekend as part of the clinical rotation

To liaise with other clinical pharmacists to ensure efficient transfer of care

To provide other duties as specified by the Clinical Lead Pharmacist for Medicine & Neonates

To be aware of, and apply, relevant legislation such as the H&S AWA, COSHH, Medicines Act, GMP etc.

At all times practice in accordance with the General Pharmaceutical Council's Standards for Conduct, Ethics and Performance

At all times, act in line with and deliver projects through the lens of the Trust's values around Equality, Diversity and Inclusion and Anti-Racism culture

The precise allocation of time and duties between the two bases will be decided and amended from time to time by discussion between the Trust and College managers. It will be determined partly by seasonal factors, so that although an approximate 60:40 (GSTT: KCL) time allocation will prevail, the time spent in either base will vary according to predictable changes in workload.
