About the Role:
As a Care Coordinator, you will be a key player in ensuring seamless care delivery. This role involves coordinating care packages, managing rotas, liaising with service users and families, and ensuring compliance with all relevant CQC regulations. You'll be joining a passionate team committed to delivering outstanding care and support.
The Ideal Candidate Will Have:
- Previous experience in a similar role withinhealth and social care, ideally inDomiciliary Care Services
- ALevel 3 qualification(or higher) in Health & Social Care or equivalent
- A solid understanding ofCQC regulations, safeguarding, and person-centred care
- Excellentorganisational, communication, and leadership skills
- Flexibility to work occasionalevenings and weekends
- StrongIT skillsand experience usingElectronic Care Management Systems
- Afull driving licenceand access to a vehicle (preferable)
