Job Summary:
As a member of the PACT multidisciplinary team, you will support this mission by carrying out rehabilitation and support functions, assisting in treatment, substance abuse services, education support, family consultation, and crisis intervention under clinical supervision. You may also be assigned a specialized role based on team needs.

Schedule:
Monday through Friday, 8am-4pm
Essential Functions:
1. Coordinate engagement, assessment, and treatment planning processes with the Team Leader.
2. Ensure completion of assessments and treatment plans, including the Critical Needs Assessment and Plan.
3. Coordinate comprehensive assessments, ensuring releases are obtained, and assist with scheduling interviews.
4. Work with the Primary Care Manager to complete clinical formulations and identify prioritized needs.
5. Facilitate treatment planning meetings, assist with goal setting, and ensure documentation and communication of interventions.
6. Provide therapeutic expertise and training to team members.
7. Engage in individual, group, and family therapy as needed.
8. In the absence of the Team Leader, assume program leadership including facilitating meetings and supporting clinical discussions.
Team Role Functions:
1. Serve as Primary Care Manager for assigned persons, developing and evaluating treatment goals.
2. Conduct assessments and provide treatment and rehabilitation services.
3. Collaborate with natural supports and participate in team meetings.
4. Share in administrative responsibilities and provide crisis assessment and intervention.
5. Complete all required documentation.
Requirements:
* Master’s Degree in Psychology, Social Work, Rehabilitation, or related field.
* Valid LICSW, LCSW, LMHC, or equivalent license.
* At least two years of experience with individuals with severe mental health issues.
* Experience in assessments, treatment planning, and clinical supervision.
* Strong communication and organizational skills.
* Proficiency in computer applications including Word and Google Apps.
* Ability to climb stairs and carry up to 40 pounds.
* Valid driver’s license and use of personal vehicle.
