Job Overview:We are working with a client in the Kirklees area who is seeking a skilled and adaptable Practice Nurse to join their team on an ongoing agency basis. The role offers flexible hours during weekdays and is ideal for nurses looking to work in a supportive, community-focused environment.
Key Responsibilities:
* Provide high-quality nursing care within a GP practice setting.
* Conduct chronic disease management clinics, with a particular focus on diabetes.
* Perform routine treatment room duties including immunisations, cervical cytology, wound care, and health screenings.
* Support health promotion and patient education initiatives.
* Work collaboratively with GPs and the wider primary care team to deliver holistic care.
* Maintain accurate clinical records in line with NMC standards.
Requirements:
* Registered Nurse with valid NMC registration.
* Previous experience working as a Practice Nurse in a primary care setting.
* Strong skills in chronic disease management, especially diabetes.
* Full UK driving licence and access to a car (essential due to travel between sites).
* Excellent communication and clinical decision-making skills.
* Ability to work independently and manage own workload.
