As a DLT Senior Mental Health Practitioner, you will work a shift pattern covering 9:00am - 8:00pm Monday to Friday and 10:00am - 6:00pm on weekends and public holidays. You will support young people and their carers presenting with mental health crises across various settings, including hospitals, CAMHS offices, online/phone, and at home. The role involves liaising with other agencies, reducing hospital presentations and admissions, and supporting discharge planning. You will act as a liaison to acute care colleagues for young people admitted to A&E or paediatric wards, providing assessments and advice for holistic care planning.
The West Sussex Duty and Liaison Team offers an exciting opportunity for Band 5/6 Mental Health Practitioners to apply their skills across various settings, with potential progression from Band 5 to Band 6 within a year.
