Job Title: Trainee Advanced Clinical Practitioner Team: Clinical Base location: King Street Walk in Centre the post holder will be required to travel to other sites to fulfil training requirements Line Manager: Clinical Education Lead Shifts: Various over 7 days Salary: £24.03+ 30% uplift after 6pm Monday to Friday &all daySaturday, Sunday & Bank holidays. Duties and Responsibilities Clinical: The post holder will undertakethe relevant courserequiredto fulfil the training post requirements The post holder will work as a trainee under the supervision of a clinical educator or an experienced member of the clinical team, to providefirst contactclinicalassessment, diagnosis andtreatment forpatients attending with undiagnosed and undifferentiated conditions. Throughout the training programme the post holder will develop the skills of an autonomous practitioner ensuring that evidence-based practice is inherent in all aspects of care and treatment Act as a referral agent and care co-ordinator by establishing multi-agency collaboration across the primary/secondary interface to meet the needs of the patient under supervision. Work under supervision with confidence and competence within professional guidelines Work within the parameters of current nurse prescribing legislationifappropriateand/orapproved Patient Group Directives toinitiatepharmacological interventions Recognise patients with changing health status and takeappropriate actions Clinically examine and assess patient needs holistically and plan clinicalcare accordingly Maintainaccurate, contemporaneous electronic patient record Research and Quality: Demonstrate evidence-based care, working within agreed guidelines and protocols Take an active role in research studies within the policy framework of the organisation. Participate in audit, datacollectionand activity monitoring to improve performance and inform future service developments. Choose an area of special interest within advanced clinical practicewith the intention to develop and take forward in yourACProle Teaching and training: Take responsibility for own development, learning and performance ensuring course requirements are fulfilled Management: Actively promote integrated working in collaboration with other health care professionals both within and outside the LCD organisation. Contribute to the development, implementation,review,and evaluation of clinical protocols. Ensure effective communication,participatingin meetings and liaising with other professionals, services and agencies asrequired. Contribute to the development of a specialist service and act as a change agent andfacilitatethe change process whilst encouraging others to be innovative and adaptable in the approach to change Professional and Educational: Develop own skills and competencies through education, training and development activities related to the current and future demands of the role, seeking opportunities to enhance skills and knowledge appropriate to the role. Understand and observe professional and national guidelines on confidentiality. Comply with the corporate governance structure in keeping with the principles and standards set by the organisation. Ensure clinical supervision needs are met. Participate in teaching and mentoring other health professionals and members of the multi-disciplinary team. To develop andmaintaina positive and quality learning environment for the clinical team by providing support, ensuring developmental requirements of junior staff are addressed Participate in annual PDR process: Maintain a portfolio of development evidenceappropriate forrevalidation purposes.
