We are looking for an enthusiastic and adaptable Community Nurse to join our caring and progressive team in Deal.

You will work as part of a close-knit team who are dedicated to the health and wellbeing of people in Deal. Deal is made up of a diverse patient group with complex needs.

District nurses make a difference by making sure our services are proactive and patient-focused, helping to keep people at home and independent for longer.

Our district nursing teams are closely aligned with local Primary Care Networks, to make sure patient care is holistic and joined up. We believe in treating the whole person, not just the condition, so we take time to get to know our patients.

Main duties of the job

District nurses provide complex nursing care to people at home and in residential care homes.

As a district nurse, you'll

1. assess the healthcare needs of patients and families
2. monitor the quality of care they're receiving
3. be professionally accountable for its delivery

Your patients could be any age, but they'll often be elderly, while others may have been recently discharged from hospital, be terminally ill or have physical disabilities.

As a Band 5 Community Nurse you will work as part of a team to keep people safe and well at home. You will use your clinical judgment to support evidence-based care planning for people living at home, in care homes or other settings and make sure that people who use our services have a good experience.

Detailed job description and main responsibilities

Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts and the Kent and Medway Integrated Care Board (ICB). Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.

Job responsibilities

Person specification

Qualifications, training and experience

Essential criteria

4. You will have a Level 6 Degree or equivalent qualification, or equivalent knowledge acquired through experience plus a diploma in nursing
5. You’ll need to have recent experience of healthcare gained through education, work experience or previous employment. This role is ideal for a newly qualified graduate nurse.
6. You will be a registered health care professional with the Nursing and Midwifery Council.

Knowledge and skills

Essential criteria

7. You’ll need have sound communication skills in order to understand and communicate both verbally and in writing.
8. You will need to be able to be able to use IT systems and databases.
9. You will understand where to signpost clients and families to alternative services to support behaviour change and improve health.

Desirable criteria

10. Having an awareness of developments within the NHS would be an advantage.

Personal Attributes

Essential criteria

11. A full, valid UK driving licence, or a full, valid driving licence issued by an EU country (not exchanged from a non-EU country licence) and the ability to travel independently are essential for this role.
