We are looking for an experienced,motivated and proactive Senior Therapy Assistant to work as part of ourEnablement Therapy team; consisting of Physiotherapists, OccupationalTherapists and Administrator.


Main duties of the job

In this role you will work closely and collaboratively with patients, those important to them and the established Multi-Disciplinary Team, by proactively undertaking, reviewing and evaluating holistic palliative assessment an rehabilitation plans of care - as allocated to you by the Occupational Therapists and Physiotherapists.

Your role will be working across our In-Patient Unit and Day Hospice, supporting our Out-patient activity as requested. Completing essential housekeeping duties and tasks to ensure the efficient running of the Enablement team service delivery.

To be considered you will need to have experience of team working in a Health and / or social care setting, possess sensitive, empathetic communication and interpersonal skills and the ability to organise and prioritise yourself and your allocated work / programmes of care accordingly.

Experience of group work and rehabilitation is desirable.

Job responsibilities

To support the provision of Physiotherapy(PT) and Occupational Therapy (OT) treatment plans and daily operationalsupport as part of the Enablement Therapy team.

Supporting patients with lifelimiting conditions to maximise physical and functional independence andimproving quality of life through provision of holistic palliative assessmentand rehabilitation.

To assist Enablement therapy team administrative and housekeeping duties to support efficient service delivery.

Clinical

Person centred collateral information gathering following handover and referral to the Enablement team as delegated to you by registered OT/PT. Sometimes being the first point of contact for the Enablement Team.

Manage and prioritise your ownsmall routine caseload to include assessment of transfers and mobility with /without aids; as allocated and delegated to you by OT and / or PT team. Workingwithin the scope of role, knowing when to seek assistance from a registered OT/ PT.

To be aware of contraindications/ precautions when carrying out any direct patient related activityindependently, reporting back concerns to senior therapists. Applying anatomy and physiology knowledgespecific to patient groups to facilitate appropriate decision making. Activelyseeking support when unsure or concerned.

Carry out delegated ADLassessments to include: personal care, kitchen skills practice with/withoutequipment, stairs and mobility assessments in line with the patients goals andtreatment plan.

To take an active role inidentified Enablement Day Therapy group sessions which may be holisticeducation, discussion or exercise based.

To assist the Enablement teamwith assessment and measurement of patients requiring complexseating/wheelchair assessments and onward treatment planning, referrals.

To be proactive and takeinitiative in ensure timely ordering / delivery of equipment for both dischargeplanning and managing onsite stock maintenance, collection/cleaning ofequipment.

To assess, demonstrate and issueroutine pieces of equipment to patients i.e. walking aids, toilet equipmentwithin your scope of practice. Appropriately documenting your outcome ofassessment and clinical reasoning / risk considerations. Initiating feedback tothe allocated OT / PT the outcome and intervention analysis in a timely manner.

To educate and advise patients,relatives / carers on the correct use of equipment, maintenance and safety,ensuring timely and responsive documentation.

To maintain timely, accurate andlegible records of all interventions with patients, using an agreed format, inaccordance with standards set by the therapy service and LOROS.

To proactively plan annual leave,training, and clinical supervision, seeking guidance from supervisor, deputy orclinical lead therapist as needed to ensure the department can continue tooffer a safely staffed service.

To take the initiative inidentifying own personal development needs through supervision and participatein an annual personal development review with supervisor. To be responsible for recording and providingevidence of own continuing professional development activities and achievementof objectives set at personal development reviews.

To be responsible fororganisation, participation and recording of clinical supervision sessions withallocated supervisor.

To attend and participate inLOROS mandatory training programmes and competency based training programmesspecific to the senior therapy assistant role.Ensuring the safe use of appropriate medical devices required to carryout assessment and intervention.


Person Specification


Knowledge

* Understanding of the role of PT and OT in palliative care
* Understanding of the role of a therapy Assistant in palliative care
* An appreciation of the principles of palliative care and the needs of patients / families with a life-limiting illness
* Understanding of own professional scope of practice
* Awareness of commonly presenting long-term conditions in palliative care
* Understanding of healthcare legislation such as safeguarding principles, mental capacity, confidentiality
* Knowledge of stock control and processes for ordering therapy equipment
* Awareness of the roles of other members of the MDT (Medical, Nursing, Social work etc.)


Experience

* Experience of working within a health and/or social care environment
* Experience of working within a team
* Experience of implementing and evaluating programmes of care as prescribed by
* qualified practitioners and working within an
* agreed scope of practice
* Experience of working as a Therapy Assistant with PT / OT
* Experience of working with patients with long term conditions
* Experience of supporting discharges from an inpatient setting


Qualifications

* Education to GCSE standard or equivalent (Maths & English)
* NVQ Level 2 in a health-related subject (or equivalent health or social care qualification)
* Basic food hygiene


Skills / Abilities

* Basic moving and handling skills
* Good interpersonal skills including listening and empathy
* Ability to communicate clearly and sensitively with patients, families and visitors who may present with cognitive and / or communication difficulties
* Ability to work effectively under pressure, prioritising own workload to respond to the needs of the service
* Ability to manage time effectively, organising oneself to work across and support both PT and OT professionals (Enablement caseload) on a daily basis
* Proficient in the use of Microsoft Office (including Word, Excel and Outlook) and administrative systems
* Working with patients with rehabilitation needs
* Knowledge of and/or a range of appropriate therapy interventions
* Working within a multi-disciplinary team
