Role Description
This is a full-time hybrid role for a Case Manager, based in Tewkesbury with flexibility for some remote work. As a Case Manager, you will be responsible for coordinating and managing individual rehabilitation cases. Daily tasks include assessing client needs, developing and executing rehabilitation plans, liaising with healthcare providers, conducting regular follow-ups, and ensuring clients are progressing toward their recovery goals. You will also document case activities and outcomes, ensuring compliance with regulatory and company standards.

* Excellent assessment, planning, and coordination skills
* Strong interpersonal and communication skills to work effectively with clients, healthcare professionals, and other stakeholders
* Experience in case management, rehabilitation, or clinical roles
* Ability to work both independently and collaboratively in a hybrid work environment
* Knowledge of health and wellbeing initiatives, along with a passion for improving client outcomes
* Strong organizational skills and attention to detail
* Relevant degree or certification in healthcare, case management, or a related field
* Experience with data insights and metrics to track and report on client progress is a plus
