We have an exciting opportunity for a HICCS Community Care Assistant to join an organisation making a real difference in peoples lives.
We're looking for a committed, reliable and experienced care support worker to join our expanding Home Independence and Crisis Support Service (HICSS).
This role is currently on a temporary basis to help with the increased demand for our services, but with the potential for the successful applicant to join our permanent team within the HICCS or domiciliary service.
Currently the hours on offer are 8am to 8pm shifts.
Responsibilities
* You’ll be supporting customers being discharged from hospital and returning home by ensuring the right care and support is provided during this period of transition including completing assessments, crisis interventions and emergency response, personal care, medication, supporting with use of daily living/rehabilitation aids.
Requirements
* Home Care experience: 2 years (required)
* Full driving licence and access to own vehicle (required); you will be expected to work across all areas of South Tyneside.
