We're looking for a passionate and dynamic Band 5 Occupational Therapistto join our exciting Intermediate Care andRapid Response Teamworking withcommunity patients and community hospital patients.
You'llbe working closelywith ateam of Occupational Therapists as well as working collaboratively with ourPhysiotherapists, Nursing and Support Worker colleagues.

Travel acrossDartford, Gravesend, Swanley and Swale Localities is required and therefor a full UK driving licence and access to a vehicle for work purposes is essential.
Main duties of the job
Within this role, you will:
1. Communicate complex and sensitive information to patients, carers, families and members of the multi-disciplinary team and/or those in other professions, from initial assessment to discharge.
2. Use communication skills to convey complex information in an easy to understand format where there may be significant barriers to understanding.
3. Work closely with patients/ clients and their carers in goal setting and decision making.
4. Use empathy, tact, sensitivity and discretion when communicating life changing events patient remaining at high risk of Rapid Response and potentially requiring 24hr care.
5. Understand and maintain the required standards of clinical record keeping in line with Organisational and professional guidelines.
6. To work in partnership with other professionals. Supporting staff, patients and carers within the Team and organisation.

As a Band 5Occupational Therapist within our Rapid Response Team, youll be part of our valued team based at LivingstoneCommunity Hospital in Dartford, North Kent.
