We are excited to offer a brand new opportunity for two EHCP (Education, Health and Care Plan) Social Care Officers on a full-time, 12 month fixed term/secondment basis.
Any successful internal candidate will be offered the position as a secondment and, in line with Section 8.1 of the Secondment Policy, must consult with their manager before applying. Search “secondment policy” on the intranet for more information.

About you
The newly created role of EHCP (Education, Health and Care Plan) Social Care Officer will play a vital part in supporting children and young people with Special Educational Needs and Disabilities (SEND), and their families, right at our front door and across Children’s Social Care teams.

As an EHCP Social Care Officer, you will have experience or training in working with children and young people with SEND and their families, along with excellent administration and ICT skills.

You will be working within our Multi-Agency Safeguarding Hub (MASH) and collaboratively across all the Children’s Social Care teams.

Key responsibilities
* Lead on social care assessments as part of the Education, Health and Care (EHC) Plan process.
* Work collaboratively with children, young people, families, schools, health professionals, and other partners.
* Ensure timely progression of EHC social care assessments and plans.
* Deliver effective interventions that support children within their families and improve outcomes.
