Hertfordshire County Council is seeking an experienced Occupational Therapist to join their Learning Disabilities & Transitions Team. This role focuses on supporting adults with learning disabilities and young people transitioning into adult services, delivering high-quality Care Act assessments and interventions to improve outcomes and promote independence. Location: Hertfordshire Rate: £36 per hour Hours: Full-time Key Responsibilities: Manage a caseload of adults with learning disabilities and individuals in transition to adult services. Complete Care Act assessments and contribute to support planning using appropriate frameworks. Build and maintain effective working relationships with service users, families, carers, and multi-disciplinary professionals. Work autonomously where required, prioritising workload to meet service demands. Act on instructions and requests, organising resources to ensure efficient service delivery. Participate in case reviews, planning meetings, and service improvement initiatives. Provide feedback on operational issues to support continuous service improvement.
