Job overview To support young people with learning disabilities as they transition from children to adult services. Predominantly working with people age 17 ½ and above The role will involve working closely with education providers, health professionals, families, and carers to promote smooth, person-centred transitions. The post-holder will provide education and support around health needs, capacity and consent, and annual health checks, and act as a key link between services. If you don't already have a Level 3 qualification or equivalent experience in a relevant subject, RDaSH are offering this post alongside the opportunity for you to complete a Level 3 apprenticeship qualification [relevant to your role]. As an apprentice within RDaSH you will be given the opportunity to gain experience, learn work-related skills and obtain all the tools needed to thrive. An apprenticeship includes regular training with a college or other training organisation. Around 20% of your working hours will be spent training or studying. This leads to a nationally recognised qualification. Advert · Under direction of senior community nurse, hold and manage a defined caseload of young people transitioning from child to adult services · Offer tailored support to each individual, promoting choice, independence, and inclusion. · Liaise with multi-disciplinary teams, families, carers, and service providers · Attend transition planning meetings and reviews. · Deliver and support health and wellbeing workshops in community settings and specialist schools/colleges. Promote awareness and understanding of health services, and healthy lifestyles. · Build strong working relationships with local specialist schools and colleges. · Provide in-reach support, including drop-in sessions, group work, and individual preparation for adult services. · Act as a visible and approachable link for students, staff, and families.
