With a particular focus on frailty, the Pharmacy team at Kent Community Health NHS Foundation Trust provides specialist medicines optimisation support and guidance to patients, carers, healthcare and social care staff across KCHFT and partner organisations. The joint post holder will support the service requirements as part of their role. Optimising a person's medicines ensures that they are taking their medicines as intended, using a person‑centred approach where shared decision‑making is an essential part of evidence‑based medicine and systems wide.
You will work on location at agreed Community Hospital sites across Kent and at GP Practices within West Kent ABC PCN, Maidstone, and remotely as agreed. This is an evolving and forward‑thinking service that supports the provision of safe and effective medicines optimisation services to cohorts of patients within these settings, with, where appropriate, an opportunity to follow patients across the Primary Care Interface into their normal place of residence.
The joint post holder will work within their clinical competencies using specialist pharmaceutical knowledge and skills to work with all stakeholders, measuring and monitoring medicines optimisation outcomes to improve and achieve the best possible outcome for every patient.

Supported delivery of medicines optimisation services across community hospital sites and GP practices, including patient identification, medication review, education and training of healthcare professionals, social care staff, and patient groups. Worked within multidisciplinary teams, contributing to audit and project management activities, and extracted and presented data from Datix and ePACT2.
