Community Staff Nurse

The School Health Services consist of School Nursing and Healthy Schools. They operate citywide across Manchester working in partnership with schools to improve the health outcomes of children, young people, and their families. Comprising of child focused functions, namely, School Health Screening; Healthy Schools; Healthy Weight Team and the Healthy Child Programme (HCP), the School Health Service contributes to tackling health inequalities by addressing the city's key public health priorities for children. The role is citywide with an expectation of all functions to be accessed to gain experience within the School Health Service.


Main duties of the job

Provide a public health-based service that contributes to improving the health of children and their families in Manchester through partnership working work within a multi-agency arena to enable children and young people to reach their full potential (physically, emotionally, and socially) through education and healthy lifestyles.

Working in partnership with families and other agencies contribute to the assessment framework for children in need/ Early Help Assessment processes.

Within the scope of professional practice and using appropriate frameworks assess, plan, deliver and evaluate the health needs of specific groups of children e.g., looked after children, children with additional needs.

Demonstrate the ability to co-ordinate and manage own workload including data collection, maintenance of records and ensuring that reporting mechanisms are in place and working effectively Complete health assessments for children and young people as required.

To contribute to the SEND agenda and EHC by supporting schools if required by providing relevant information to compile care plans and to co-ordinate care plan involvement in supporting the child in school or other settings

Person Specification


Qualifications

* Registered Nurse (NMC)


Experience

* Nursing experience of working with families/children in either acute or community
* Experience of working within a skill mix team
* Interagency working
* Working on own initiative
* Experience and evidence of safeguarding
* Experience of planning and carrying out health promotion programmes


Skills

* Able to organise, prioritise and delegate
* Able to present information, written and orally in a clear logical manner
* Excellent interpersonal communication skills
* IT skills - including use of email, clinical databases, word processing
* Evidence of ongoing professional development


Knowledge

* Knowledge of safeguarding, policies, procedures and practice
* Principles of confidentiality and consent
* Knowledge of national and local strategies relating to the school nursing service
* Health needs of a culturally and ethnically diverse urban population with high levels of deprivation
