Mill Bank Surgery is a friendly Surgery located in the centre of town with very good transport links.

We are looking torecruit an experienced Practice Nurse for 15 hours per week on a Wednesday andFriday The position is a permanent post, and you will be working alongside ourexisting supportive and friendly clinical and administrative teams.

The hourly rate for thisposition will be dependent on your experience


Main duties of the job

You will be responsible for thedelivery of nursing services, delivering care to our practice population of 10,500patients. We are looking for experience in long term conditions management inline with QOF, such as Diabetes, Asthma, Coronary Heart Disease, and also otherareas such as Cytology and Childhood immunisations.


About us

Our clinical team consists of 3GP Partners, 3 Salaried GPs, 2 Advanced Nurse Practitioners, 2 Practice Nurses and 1 Healthcare Assistants.

We also benefitfrom PCN additional roles including 2 Clinical Pharmacists, 1 Pharmacy Technicians, 3 First Contact Physiotherapists, a Social Prescriber and a Mental Health Practitioner. We are supported by a resilient and longstanding administrativeteam.


Job responsibilities

The Practice Nurse is to support new-to-practicenurses and other healthcare professionals within the organisation and acrossthe wider network to enhance integration with community, secondary, and socialcare services.

This role is responsible for the delivery of evidence-basednursing services, working as part of the practice multi-disciplinary team,delivering care within their scope of practice to the entitled patientpopulation.

Working autonomously, the PracticeNurse will be responsible for a number of clinical areas such as public healthand screening programmes, including infection prevention and control, chronicdisease management, immunisations, health promotion and prevention, wellman/woman clinics and cervical screening, Furthermore, they embed populationhealth management approaches within general practice, targeting healthinequalities and improving access.

The post-holder will be an integralpart of the general practice team, as well as supporting the wider practicenurse network and multi-disciplinary team. They are to participate in formaltraining events and to promote best practice in their area of expertise as wellas supporting the practice management team in the reviewing and delivery ofclinical policy and procedure.

Thisorganisation must ensure that the post-holder has access to appropriateclinical supervision and an appropriate named individual in the organisation toprovide general advice and support on a day-to-day basis. Person Specification


Experience

* Broad knowledge of general practice and experience of working as a Practice or Community Nurse
* Experience with chronic disease management
* Experience of working autonomously
* Experience of infection prevention and control measures
* Experience of managing elderly and housebound patients
* Ability to communicate complex and sensitive information effectively with people by telephone, email and face to face
* Understanding of safeguarding adults and children
* Understanding and knowledge of healthcare provision in GP surgeries, QOF and enhanced service
* Broad knowledge of clinical governance
* Knowledge of public health issues in the local area
* Awareness of issues within the wider health arena and knowledge of health promotion strategies
* Knowledge of national standards that inform general practice (NSF NICE guidelines)


Qualifications

* Registered Nurse (Nursing and Midwifery Council)
* Has evidence of working at the relevant level of practice as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework
* Childhood Immunisations
* Cytology
