Connect2Halton is currently recruiting for an experienced Qualified Social Worker to join the Short Break Service for Children with Disabilities Team within Halton Council. We are looking for a Social Worker who shares our commitment to provide services that enable families of children with disabilities to have an improved quality of life within their communities. You will have a key role in assessing the needs of disabled children who have complex needs, producing a robust, person centred, Child in Need plan and supporting them to access short break services. Key Responsibilities: Undertake complex assessments for children and young people with disabilities, identifying needs and risks, and determining appropriate short break interventions.
