Role Summary As a Support Worker, you will play a key role in helping individuals lead independent, meaningful and fulfilling lives. You'll provide person-centred support tailored to each individual's needs, assisting with personal care, daily living activities, appointments, and community engagement. Through your support, you'll promote independence, wellbeing, dignity, and safety while ensuring all care is delivered in line with safeguarding responsibilities and CQC standards. Key Responsibilities Person-Centred Support Deliver high-quality, person-centred support to individuals with learning disabilities, autism, mental health, sensory, complex and physical support needs. Promote independence, confidence, life skills, and positive outcomes. Provide personal care with dignity, respect, and compassion. Build positive relationships and engage meaningfully with the people you support. Read and follow all care plans, risk assessments, and support guidance using the applicable electronic care reporting software. Maintain professional boundaries, including no personal phone use whilst supporting clients. Health & Wellbeing Support with medication administration in line with training and policy. Monitor clients and report changes in health, wellbeing, or behaviour. Support attendance at appointments and community activities. Ensure personal care supplies and toiletries are maintained within the home. Documentation & Compliance Complete accurate and professional records the applicable electronic care reporting software. Record incidents, medication, daily notes, and outcomes clearly and promptly. Follow safeguarding procedures and report concerns immediately. Work in line with CQC standards, GDPR, and company policies. Teamwork & Flexibility Work flexibly across a 24-hour service, including evenings, weekends, nights, and bank holidays. Support across services where required. Attend training, supervision, and team meetings. Work positively with colleagues, families, and professionals. Person Specification Essential Previous experience in care; supported living, day centre services, community engagement, etc. A caring, proactive, and person-centred approach. Good communication, IT, and written recording skills. Understanding of safeguarding and professional boundaries. Flexible, reliable and able to work independently and as part of a team. NVQ/QCF Level 2 or 3 in Health & Social Care. Full UK Drivers Licence (manual preferred). Desirable Experience within a CQC-regulated service. Medication administration experience.
