You will be an experienced practice nurse supporting with all aspects ofnursing care and improving on the safety and quality of patient care at everyopportunity.

To maintain a high professional standard of nursing care in adherence toall Practice protocols/policies & guidelines, as well as to provide nursingassistance to the GPs and other members of the primary healthcare team.

You will be supported by the clinical team and work with the widerclinical team to deliver holistic care to our patients.

We will encourage and support your continued professional development aswell as assisting in developing a specialist area of interest where possible.


Main duties of the job

Provide ahigh standard of nursing care

Provide Chronic Disease Managementand screening in line with current evidence based guidelines

Ensurevaccines are administered under Patient Group Directives

Provide a holistic approach andlifestyle advice on diet, smoking, alcohol intake and excercise

Recognition of the limitations ofyour own safe practice and when to ask for additional clinical support

Good communication skills whendealing with patients and colleagues

Excellent organisational skills andthe ability to manage your own caseload

Contribute to the achievement oflocal and national targets QOF & IIF (PCN DES) indicators


About us

Comeand join our high achieving very friendly GP training practice. Supported by 1GP partner, 3 Salaried GPs, 3 highly skilled and experienced Nursepractitioners, 1 Advanced Practitioner, 1 Practice nurse, 2 HCAs and in-housePractice pharmacist and Prescribing technician, along with our very experiencedand supportive admin and reception team.


Job responsibilities

To assist and perform routinenursing tasks related to patient care, such as routine BP checks, ECG, sutureand clip removal, BMI and lifestyle advice..

To support and signpost patients toimprove and maintain their mental, physical, behavioural health and wellbeingby using behaviour change interventions, such as undertaking health checks(physical and mental),

To undertake adult NHS immunisationprogrammes and ensure vaccines/injections are administrated under patient groupdirectives or patient specific direction.

To undertake administration ofprescribed medicines via oral, IM, SC routes, following competency basedtraining and practice policy.

To undertake the collection ofpathological & microbiology specimens including venepuncture, swabs, urineculture in line with local guidelines

To support chronic diseasemanagement including monitoring patients with stable asthma, undertakingpreliminary investigations for Type 2 diabetes patients, such as foot checks.

To provide general health promotioneducation, advice on national screening programs and guide/refer patients toappropriate services.

In line with national guidanceundertake screening programs such as cervical screening tests.

To undertake any investigatorytest, such as Ambulatory BP monitoring, automated Dopplers and Spirometry,taking basic history and referring patients to relevant clinician forinterpretation/ planning of care.

To be able to provide appropriateself care advice for patients presenting with minor illness within own level ofcompetency, referring to GP/ANP whereappropriate.

Chaperone and assist patients whereappropriate who are being examined by another clinician.

To assist GPs with minor surgeryand well women procedures.

To maintain effective infectioncontrol measures in line with local guidelines and practice policy.

To be able to recognise and manageanaphylaxis, according to the current UK Resuscitation Council guidelines.

To be able to performCardio-pulmonary resuscitation (BLS) and the correct use of AutomatedDefibrillator following training, according to the current UK ResuscitationCouncil guidelines.

To engage in the support thetraining of nursing and medical students, and newly appointed colleagues.

Qualifications

* Registration as a Registered General Nurse
* Registration within the NMC


Other

* Occupational Health Clearance


Experience

* Experience working in a Healthcare setting
* Experience of working as a Practice Nurse
* Experience of Chronic Disease management
* Excellent interpersonal skills
* Effective written, verbal & non-verbal communication with patients/ carers & all members of multidisciplinary teams
* Demonstrate IT literacy and competence with e-mail, word, intranet etc.
* Effective time management
* Evidence of and willingness to undertake quality improvement
* Knowledge of health and safety in workplace
* Experience of working in Primary Care or Community setting
* Knowledge of clinical systems


Knowledge

* Understanding of Data Protection Act & Patient Confidentiality
* Knowledge of NMC Code of Conduct & Professional Accountability
* Knowledge of Clinical Skills relevant to Primary Care
