About The Team We Are Recruiting To

This is an exciting opportunity to work in the Coventry Community Mental Health Team. You will be working alongside Coventry and Warwickshire Partnership Trust colleagues to provide a seamless service to citizens with severe and enduring mental health challenges. The team is a multidisciplinary service, and we work closely with Social workers, nurses, occupational therapists, psychologists and psychiatrists to deliver person-centred care to our citizens. What is the job role?
The Community Support Worker is an integral part of the social care service which is delivered as a member of a multi-disciplinary team for people with mental health needs, their family groups or carers, within the community and in-patient mental health facilities.
What You’ll Be Doing
* Carry out assessments under the Care Act 2014 to identify unmet social care needs, focusing on strengths and recovery
* Develop and implement person-centred care and support plans for citizens and carers
* Review and evaluate the effectiveness of care plans and services
* Provide crisis support and contribute to daily team handovers
* Attend multi-disciplinary meetings and collaborate with internal and external teams
* Liaise with statutory, voluntary, and independent sector agencies to coordinate care
* Maintain accurate records and documentation in line with policy and procedures
* Participate in team meetings, supervision, and ongoing professional development
