An exciting opportunity has arisen for an enthusiastic, dynamic, and committed Nurse Practitioner to join our Award-Winning Norfolk Forensic Community Learning Disability (NFC-LD) team. This is a fantastic career development opportunity within the Adult Forensic Learning Disability community service, working alongside an established and experienced team of clinicians and practitioners to contribute to high-quality service provision, including consultation and advice, specialist risk assessments, assessments and formulation, therapeutic interventions, and training delivery, all within the service’s policies and framework.
You will be actively involved in supporting plans for accelerated transformation in forensic learning disability services, focusing on improving continuity of care for patients. This approach aims to reduce hospitalization lengths and minimize the need for inpatient admissions.
This post is based within the Norfolk Forensic Community Learning Disability (NFC-LD) Team, with close collaboration with the Broadland Clinic, a Medium Secure Forensic Learning Disability Inpatient Service within the Trust. The team will be involved in discharge planning processes.
The Integrated Community Forensic Learning Disability team is multidisciplinary and offers:
1. Training on Forensic Learning Disability topics
2. Consultancy and advice to community teams and inpatient units
3. Specialist risk assessments
4. Community-based therapeutic interventions
5. Continuity for individuals admitted to inpatient settings
6. Monitoring and proactive discharge planning
7. Active case management for high-risk forensic service users
8. Delivery of evidence-based practices for adults with learning disabilities requiring forensic services
