Overview
We are excited to offer a community CNS position at Meadow House Hospice to support the new North West London model of specialist palliative care. The role involves delivering exceptional palliative care in a specialist hospice setting, collaborating with a multidisciplinary team to enhance patient experiences and access to timely care.
Responsibilities
* Evaluate individual patient needs through assessment, history taking and clinical judgment.
* Manage a patient caseload, ensuring high-quality palliative care and appropriate documentation.
* Build strong working relationships with primary care teams and other healthcare professionals, using excellent communication and interpersonal skills.
* Exercise personal professional autonomy and make critical judgments to meet role expectations.
* Demonstrate leadership, innovation and motivation in the delivery of palliative care services.
Qualifications
* Registered nurse with experience in palliative or community nursing.
* Strong clinical skills in assessment, care planning and complex patient management.
* Excellent communication and collaborative working abilities.
* Leadership and motivation skills, with a commitment to improving patient outcomes.
