Job Overview
We are seeking a dedicated and compassionate Care Coordinator to join our team. The Care Coordinator plays a vital role in ensuring that patients receive the necessary support and services throughout their care journey. This position requires excellent organisational skills and the ability to communicate effectively with both patients and healthcare providers. This role demands strong communication and problem-solving skills, along with a dedicated commitment to enhancing the well-being of those receiving care.
Responsibilities
* Coordinate patient care by liaising with professionals, service users, and their families to ensure excellent communication and service delivery.
* Allocate calls across all geographical areas to meet the needs of the organisation using the digital platforms available to ensure and reduce travel where possible.
* Prepare reports and documentation as required for internal reviews or regulatory compliance.
* Work with other members of the allocation team on a daily basis to prioritise tasks for the day and where required report issues to senior member of staff and/or request additional support if necessary.
* Participate in the On Call Duty which includes evenings and weekends.
* Provide emergency On Call and Care cover in times of illness and holidays.
* Taking new Service Users referrals from social workers and private Service Users.
* Answering and monitoring incoming telephone calls.
* To maintain strict confidentiality and to safeguard all personal data in accordance with GDPR and Data Protection regulations.
* Maintain an organised office environment, ensuring that all administrative tasks are completed efficiently.
* Participate in team meetings to discuss patient care strategies and improve service delivery.
* Undertake care work to ensure consistent cover of calls when necessary. Additionally, whilst on Call, if no other care workers are available, to undertake calls on a 'rapid response' basis as required.
* Attend mandatory refresher training/skills to maintain and extend own knowledge through training.
Upon satisfactory completion of the six months probationary period
* To undertake staff supervision, spot checks, direct observation, appraisals and support the staff team including assessment of competence for staff undertaking the Care Certificate working alongside the Training Manager where required.
* To have oversight of the medication requirements of service users in a designated geographical area ensuring the compliance with the company Medication Policy and maintaining associated documentation.
Requirements
* Previous experience in a similar role is preferred but not essential.
* Strong organisational skills with the ability to manage multiple tasks simultaneously.
* Excellent verbal and written communication skills, with a focus on providing exceptional customer service.
* Proficiency in office software applications (e.g., Microsoft Office Suite) is essential.
* Ability to work collaboratively within a team while also being self-motivated.
* A compassionate attitude towards service users and a commitment to enhancing their care experience.
