We are looking to recruit a Reablement Co-ordinator on a permanent basis on a -hour contract who truly care and are committed to making a difference in an individual’s life.
Being a Reablement Coordinator means supporting people in their own homes for a short space of time to assist them to reach their full potential, including managing aspects of daily living skills, setting clear and achievable goals to help people rebuild independence and confidence to sustain living at home and in their communities.

You will work within the reablement team to deliver high-quality services aimed at promoting and sustaining people’s independence. You will supervise staff and mentor personal development of Reablement workers.
A driving licence is essential for this community-based role.
Ensure service users' journeys are well-coordinated and compliant with organisational policies, procedures, and Care Quality Commission guidance. Strive to achieve the best quality outcomes for service users, their carers, and families.
