Croydon serves a residential population of approximately 370,000, with 27% of these being within 0-18 years of age. Over the last 5 years emergency admissions for children and young people with asthma and wheezing has risen and is the second highest reason for hospital admissions in Croydon. There are currently 3,751, 0-17 years who have a diagnosis of asthma.

As parts of the paediatric asthma nurse specialist team you will be working alongside other asthma nurse specialist in order to deliver and maintain the best possible treatment plans for children with asthma in croydon. You will also work along side the other paediatric areas such as Rainbow childrens unit, Paediatric emergency department and other nurse specialists.


Main duties of the job

The ultimate objective of the service is to deliver a children's asthma service which will improve outcomes in asthma management by increasing children, young people and their carer's confidence to manage with the condition and improve self-management skills. All interventions will be in the majority of cases a single visit on a 1-1 basis. This will not be a case management service. The focus will be on prevention of ill health, through education and effective self-care.

The post holder will be expected to work across community, primary and acute care settings as an expert in this clinical field, to deliver a specialist asthma service to children, young people and their families. The post holder will facilitate multi-professional working and education across all sectors.

The service will be delivered Monday to Friday at times suited to meet the needs of the population cohort. The post holder will work 37.5 hours a week.


About us

Croydon Health Services provides integrated NHS services to care for people at home, in schools, and health clinics across the borough as well as at Croydon University Hospital and Purley War Memorial Hospital.

CUH provides more than 100 specialist services and is home to the borough's only Emergency Department and 24/7 maternity services, including a labour ward, midwifery-led birth centre and the Crocus home birthing team.

Purley War Memorial Hospital (PWMH) in the south of the borough offers outpatient care, including diagnostic services, physiotherapy and ophthalmology services run by Moorfields Eye Hospital, alongside an onsite GP surgery.

Our experienced district nursing teams, Allied Health Professionals and community matrons look after for people of all ages across Croydon and our Children's Hospital at Home cares for children with long-term conditions without them having to come to hospital.

Job responsibilities

* To be effective in organising and planning the day to day management of the Childrens Asthma team, ensuring the individual needs of children/young people and their families are met.
* To have an open minded approach to work and demonstrate ability to multitask in order to get the job done through flexible hours and locations
* To identify and manage risks to patients and staff.
* To develop and deliver innovative approaches to self-management education for the different age groups covered by the service.
* To disseminate best practice in the management of children and young people with asthma in the community to all appropriate community clinicians

To work closely with GP surgeries, to ensure treatment complies with best practice.

To work closely with colleagues in Public Health to identify and target hard to reach groups, with a strong focus on health outcomes and reducing health inequalities for children with asthma.

* To be responsible for line management, support and mentoring of colleagues.
* To work collaboratively and develop effective relationships within the multi-disciplinary team and other departments and organisations.
* To develop and monitor quality initiatives to ensure the delivery of quality nursing care, in line with the CHS clinical governance framework. To work strategically to influence the development of the service.

External applicants will be required to do maths and english exam before the interview


Person Specification


Education and qualifications

* RSCN/Child Branch
* Holds recognised asthma qualification or significant documented experience
* ENB 998 or equivalent
* Respiratory qualification Physical assessment skills Independent prescriber


Experience

* Experience in asthma management within primary care or community/ hospital setting.
* Experience of managing a defined caseload and ability to work under pressure to manage work load and meet deadlines
* Working knowledge of current policies and documents relating to best practice for children & young people with asthma.
* Experience of effectively transitioning young people from Children to adult services.


Skills

* Involvement in research and audit
* Ability to assess children independently and manage care plans
* Independent prescriber
* Advanced assessment skills
