Main duties of the job
Key Responsibilities
The Primary Care Network (PCN) Clinical Pharmacist has the following key responsibilities in relation to delivering health services:
* Workas part of a multi-disciplinary team in a patient-facing role to clinicallyassess and treat patients using expert knowledge of medicines in specificdisease areas.
* Provideleadership on person-centred medicines optimisation and quality improvement, whilstcontributing to the Quality and Outcomes Framework and enhanced services.
* Providespecialist expertise in the use of medicines whilst helping to address both thepublic health and social care needs of patients at the organisation and helpin tackling inequalities.
* Beresponsible for the care management of patients with chronic diseases andundertake clinical medication reviews to proactively manage people with complexpolypharmacy.
There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels.

Coastal Fareham and GosportPrimary Care Network was formed in 2019, bringing together three established independent practices:
* Lockswood Surgery
* Stubbington Medical Practice
With a patient population of approximately 39,000 we serve a largely suburban area between Portsmouth and Southampton. We are equidistant from Queen Alexandra Hospital and Southampton General Hospital, allowing us to collaborate with major local healthcare providers.
Our mission is the practices within the PCN will work collaboratively, whilst maintaining their independence, to provide additional services for all our patients. We aim to empower patients to improve their own health and wellbeing.
Job responsibilities
Through structured medication reviews, support patients to take their medications to get the best from them, reduce waste and promote self-care.
Play apositive part in supporting further integration of general practice with thewider healthcare teams (including community and hospital pharmacy) to help improvepatient outcomes, ensure better access to healthcare and help manage generalpractice workload.
Developrelationships and work closely with other pharmacy professionals across thewider health and social care system, integrated neighbourhood teams andlocality.
To act asthe point of contact for all medicine related matters, establishing positiveworking relationships.
Toconsult patients within defined levels of competence and if appropriate, independentlyprescribe acute and repeat medication.
Toreceive and resolve medicines queries from patients and other staff.
Todeliver long term condition clinics, particularly for patients with complicatedmedication regimes and, prescribe accordingly.
Toprovide pharmaceutical consultations with patients with long term conditions asan integral part of the multidisciplinary team.
Toreview medications for newly registered patients.
Toimprove patient and carer understanding of confidence in and compliance withtheir medication.
Tomaintain accurate clinical records in conjunction with extant legislation.
Toencourage cost-effective prescribing throughout the organisation.
Toembed a robust electronic repeat prescribing system.
Toprovide advice and answer medication related queries from patients and staff.
Respondto the organisations medicines optimisation systems, including the repeatprescribing and medication review systems.
Toimprove the quality and effectiveness of prescribing through clinical audit andeducation, to improve performance against NICE standards and clinical andprescribing guidance.
Todevelop yourself and the role through participation in clinical supervision,training and service redesign activities.
Toprovide subject matter expertise on medication monitoring, implementing andembedding a system.
Toactively signpost patients to the correct healthcare professional.
To reviewthe latest guidance, ensuring the organisation conforms to NICE, CQC etc.
Toprovide targeted support and proactive reviews for vulnerable, complex patientsand those at risk of admission and re-admission to secondary care.
Tosupport in the delivery of enhanced services and other service requirements onbehalf of the organisation.
Toparticipate in the management of patient complaints when requested to do so andparticipate in the identification of any necessary learning brough aboutthrough clinical incidents and near-miss events.
Toundertake all mandatory training and induction programmes.
Tocontribute to and embrace the spectrum of clinical governance.
Tocontribute to public health campaigns (e.g., COVID-19 or flu clinics) throughadvice or direct care.
To always maintain a clean, tidy and effective working area.
Secondary Responsibilities
In addition to the primary responsibilities, the Clinical Pharmacist may be requested to:
* Support delivery of QOF, incentive schemes, QIPP and other quality or cost effectiveness initiatives.
* Agree and review prescribing formularies and protocols and monitor compliance levels
* Improve the data quality of medicine records and linking to conditions.
* Deliver training, mentoring and guidance to other clinicians and staff on medicine issues.
* Working in partnership with pharmacists and clinicians at place level, improve the safety and quality of prescribing after discharge from hospital admissions and attendance.
* Provide leadership and support to prescription administrative staff.
* Work with community pharmacists, hospital pharmacists and other stakeholders in the medicines supply chain to improve patient experience and manage incidents.
* Support virtual and remote models of consultation and support, including e-consultations, remote medication review, telehealth and telemedicine.
* Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner. Duties may vary from time to time without changing the general character of the post or level of responsibility.

The post-holder will ensure all members of staff complywith the member practices health and safety policies.
Ensure that site risk assessments and infection controlaudits are completed in line with practice policies and comply with currentHealth and Safety legislation.
Ensure any health and safety concerns are reported to thePractice and PCN Transformation Manager.
Maintain up to date knowledge of health and safety andinfection control statutory and best practice guidelines and ensureimplementation across the business.
Making effective use of training to update knowledge andskills and initiate and manage the training of others.
Actively identifying, reporting, and correction of healthand safety hazards and infection hazards immediately when recognised.

Experience
* An appreciation of the nature of General Practice and Primary Care Networks.
* An appreciation of primary care prescribing concepts of rational prescribing and strategies for improving prescribing.
* Experience and an awareness of common acute and chronic pain conditions that are likely to be seen in general practice.
* A minimum of two years working as a pharmacist demonstrated within a practice portfolio.
* Experience in managing pharmacy services in primary care.
* In-depth therapeutic and clinical knowledge and understanding of the principles of evidenced based healthcare.
* Understanding the mentorship process.
* An appreciation of the new NHS landscape, including the relationships between individual practices, PCNs and the commissioners.
