Community Mental Health Practitioner - Dover
Invicta Health CIC, in partnership with Kent and Medway NHS and Social Care Partnership Trust (KMPT), seeks Community Mental Health Practitioners to be part of Mental Health Together. This role is in Dover, Kent.

Job Responsibilities
* Foster recovery through empowerment of service users and carers using a strengths-based approach.
* Provide a range of evidence-based interventions and risk management to reduce symptoms and high-risk behaviours during relapse.
* Be proactive and creative in delivering care to support service user recovery.
* Engage service users and carers in relapse prevention and risk management plans; support discharge through appropriate pathways.
* Deliver flexible, responsive care; work with MDTs and partner agencies to ensure appropriate care pathways.
* Deliver care within a Strengths-Based Model in the community; support carers and families; educate service users and carers on mental health issues.
* Respond swiftly to urgent referrals and prevent admission or readmission where possible.
* Maintain high quality record keeping and documentation; ensure care plans reflect current needs and preferences.
Leadership and Professional Practice
* Participate in team development, mentoring, supervision, and governance; promote integrated working internally and externally.
* Act as a positive role model; support induction and ongoing professional development.
* Adhere to professional codes of conduct (NMC/HCPC) and maintain relevant registration.
Clinical Skills and Qualifications
* Registered Mental Health Nurse or Registered Occupational Therapist (diploma or degree).
* Mentoring and Enabling in a Clinical Setting (MELPS) desirable.
* Minimum of 2 years post-registration experience; prior community team experience preferred.
* Current UK professional registration required.

Location and Requirements
Location: Coleman House, Brookfield Avenue, Dover, CT16 2AH. Salary: £37,338 to £44,962 per year. UK right to work in the UK required. Role subject to DBS check.
