Description: Part-time Performer Dentist required for 3 days per week in a busy, mixed, INDEPENDENTLY OWNED practice in Maccelsfield, Cheshire
• Mon to Weds - MUST BE AVAILABLE ALL 3 DAYS
• EXCELLENT PRIVATE EARNING POTENTIAL
• 50% NHS - 50% PVT patient list
• Practice Plan and FPI patients
• From £12 per UDA
• 45% split on PVT
• 50/50 labs
• Fully computerised surgeries
• Brand new digital x-ray
• iTero Scanner
• Rotary Endo
• Practice will provide any other equipment required by the Associate
• Hygienist on-site
• Parking on-site

Compliance requirements:
- Full NHS Performer Number if applicable
- Local Area Team confirmation
- GDC certificate
- UK Blood test report showing Hep B Immunisation and Titre levels, Hep C, HIV and TB results dated within last 5 years
- Indemnity certificate
- Passport
- Enhanced DBS (within the last 12-36 months)
- Clinical references covering places of employment over past 2 years (minimum 2 referees)
- Degree certificate
- CPR/AED/Medical Emergencies certificate dated within the last 12 months
