North East Ipswich PCN is looking to welcome a qualified and experienced clinical pharmacist into our busy practices, to play a full and active role as part of the practice multi-disciplinary team. This position will be offered as a 12 month contract, on a full-time basis, but part-time can be considered. Remote working can be considered. The main responsibilities of the role include: Clinically assessing and treating patients, using expert knowledge of medicines, for specific disease areas. Providing specialist expertise in the use of medicines and medicine information (i.e. side-effects, dosage) across the practice. Providing pharmaceutical medication review, and health education advice, to patients at the designated practices by face-to-face, telephone or notes-based review, as agreed with the practice. Providing leadership on person-centred medicines optimisation and quality improvement. Contributing to public health campaigns, including flu vaccinations, childhood immunisation and adult immunisation programmes. Processing specialist treatment letters. Supporting practice dispensary. Processing hospital medication discharge letters. Acting as a key contact in the practice, for all medication related queries. Supporting improvements in clinical care, through practice-based audits and implementing change. About the Candidate To be considered for this position, you will need the following; Qualified pharmacist with 2 years’ experience, current registration with General Pharmaceutical Council. Primary care experience desirable. Confident in use of electronic records and systems; databases and spreadsheets. NMP, or working towards NMP. Diploma in clinical pharmacy or pharmacy practice. Leadership activities in previous work. Evidence of consistent pattern of learning from education, training and experience. SystmOne experience desirable.
