We have an exciting opportunity for a dedicated, passionate, and driven individual to join our Caregiver team at Uplands House Care Home in Fareham. Uplands is a nurse-led service for people with mental health and physical health needs, located on the outskirts of Fareham.
Ideally, you will have experience in a Care Home setting, working to deadlines and within set guidelines. Full training will be provided. You should have high attention to detail and the ability to follow instructions with minimal supervision.

Key Responsibilities:
* Assess, plan, implement, and evaluate Support Plans & Risk Assessments as a Named Nurse.
* Review risk management monthly, especially on resident admission and at appropriate intervals. Support residents with daily living, following support plans.
* Document all interventions clearly and comprehensively.
* Communicate effectively with the Multi-Disciplinary team to meet residents’ needs.
* Administer and document medicines according to policies, legal requirements, and NMC standards.
* Ensure a safe environment for residents, visitors, and staff in collaboration with the team.
* Contribute to preventing and managing challenging behaviour following least restrictive practices.
* Participate in supervision, annual appraisals, and competency assessments.
* Maintain PIN through revalidation.
* Promote the good reputation of the Care Home with Nursing.
* Act as a positive role model, demonstrating professionalism and a positive attitude.
* Report and document incidents, escalating as necessary.
* Participate in reviewing incidents and learning from them.
* Work within Safeguarding Adults policies, recognizing signs of abuse, and reporting concerns.
