Your role and responsibilities
* Engage with customers to assess risk and understand their views on how interventions should be progressed.
* Gather and analyse information from partner agencies, review records, and collaborate with professionals to support effective multi‑agency decision‑making.
* Contribute to and attend strategy meetings, identifying additional interventions where appropriate.
* Respond to calls on the Professional Consultation Line, providing advice and guidance to professionals on referral pathways.
You will need
* Knowledge of adult social care, including safeguarding practice, and an understanding of the Care Act.
* Ability to work in an organised, efficient and responsive way, managing competing priorities.
* Confidence in using case management systems, ensuring accurate and timely recording.
* Strong communication skills, with the ability to adapt your approach to a wide range of audiences.
