Overview
We are seeking an experienced Physiotherapist to join the North Community Therapy Team, a friendly and supportive team of Therapists and Therapy Assistants providing domiciliary rehabilitation service to patients. You will develop your expertise treating a range of problems related to elderly care, falls, palliative care, orthopaedic, neurological and respiratory conditions and the opportunity to develop your leadership/management skills.
The post would suit an established B6 therapist or an experienced B5 with interest in community working. Successful applicants need to be efficient workers, good communicators and confident working as an autonomous practitioner. Responsibility for managing your own caseload and working effectively within the multidisciplinary team to ensure an integrated approach to patient care - preventing unnecessary admissions, promoting functional independence and facilitating timely discharge from hospital.
Responsibilities
* Manage your own caseload and work effectively within the multidisciplinary team to ensure an integrated approach to patient care.
* Identify patient needs, triage appropriately, and refer/liaise with the correct services.
* Provide specialist therapy assessment of patients, using analytical and judgment skills.
* Provide appropriate patient-centred treatment using evidence-based practice where possible.
* Devise effective care plans for each patient with specific therapeutic knowledge, recognising the patient as an individual.
* Deliver the therapeutic treatment programme or direct the intervention as necessary through Support Workers or other members of the multidisciplinary team.
* Facilitate safe discharge at the optimum point of care for patients, including assessing risk, undertaking home assessments if appropriate, ordering equipment and providing appropriate advice for patients/carers.
* Work under the supervision of the Clinical Team Lead, receiving specific therapy clinical supervision and development support from the district leads.
* Liaise with a wide range of clinical colleagues including UCR team, District Nurses, Social Care professionals and GPs to support patient-focused, coordinated case management.
Qualifications and requirements
* Current registration as a Physiotherapist and appropriate professional qualifications.
* Experience at B6 level or an experienced B5 with interest in community working.
* Efficient, good communicator, able to work autonomously and manage own caseload.
* Current driving licence and access to a car for work purposes; free onsite parking and travel expenses paid for work-related travel.
