Are you highly motivated, dynamic, enthusiastic nurse with a love for caring for patients with Haematology conditions. Then joining MYTT's Haematology CNS team is the role for you.
The CNS team comprises of 1 band 7 and 2 band 6 clinical nurse specialists who work closely with the MDT, chemo day units and our Haematology ward (Gate 21 at Pinderfields General Hospital). The CNS team work on all our 3 sites at Pinderfields, Pontefract and Dewsbury Hospital. You will act as a key worker for patients from diagnosis and through there treatment. You will play a pivotal role in the patients journey by being a point of contact offering support and advice for a variety of treatments and complex interventions.

Main duties of the job
The post holder will organise and manage a caseload for patients diagnosed with Haematological conditions, providing specialist nursingsupport, advice and education and care. You will carrycontinuing responsibility for the assessment of needs, development,implementation and evaluation of programmes of care and the setting ofstandards within the defined service group. The post holder would need to organises, prioritises and evaluate workload based on accepted referral criteria and discharge criteria in linewith operational protocols. Provide clinical, social andpsychological support to your patients using excellent communication skills,education and health promotion to enable them to reach their optimum level ofindependence and health. The post holder would be required to support the band 7 team leader to develop the service and protocols within it whilst working closely with the MDT.
You would be required to review, interpret and act upon diagnosticinvestigations and point of care testing which may effect, change to individualcare plans, liaising with MDT where necessary. This may be through nurse led clinics which would then requireliaising with Chemo Day Units and Medical Infusion Unit staff to ensure patients receive their treatment in a timely manner.

Key Responsibilities
1. Will have continuous delegated responsibility for a caseload of patients and will provide an effective and patient focused service which is considerate on needs of users and providers.

8. Promote programmes of health education and health promotion relevant to service.
9. Work within national policy frameworks relevant to the service and trust local guidance.
10. Review, interpret and act upon diagnostic investigations and point of care testing which may effect, change to individual care plans, liaising with appropriate medical staff where necessary.
11. Deal effectively and professionally with patient / service user issues collaborating with PALS as appropriate and take a proactive approach to resolving issues and learning from complaints and escalate when unable to resolve.
12. To maintain accurate appropriate and concise patient records within agreed databases and electronic systems.
13. Contribute to the multi-professional approach in the management of patients / carers within the specialist area in relation to ongoing care needs.
14. Act as a resource for specialist knowledge for clinical staff on wards / departments.
15. Ensure effective multi- disciplinary communication.
16. Support the management function of the department by provision of reports and information as required.
17. Provide mentorship and preceptorship as required to junior staff in the team or students on placement.
18. Ensure you work within an agreed divisional work plan.
