Support Coordinator

About the role
As a Support Coordinator, you’ll play a vital role in helping individuals rebuild their lives in a supported accommodation setting. You’ll provide recovery-focused support, advice, and guidance to people on their journey to stability and independence. No two days are the same — from leading 1:1 sessions and group work to coordinating clinical appointments and collaborating with volunteers, your work will be hands-on, rewarding, and transformative.
Responsibilities include:
* Managing a caseload of service users, delivering psychosocial interventions.
* Working within a multi-disciplinary team for assessments, risk assessments, and referrals.
* Facilitating group sessions with diverse individuals.
* Collaborating with partnership agencies to ensure continuity of care.
Please note you will be required to provide on-call cover 1 in 8 weeks.
About you
We are looking for someone empathetic, motivated, and committed to making a difference. Experience in support work or a related field is preferred but not essential; a drive to support people with compassion and professionalism is key.
