37.5 hours per week
Latham House Medical Practice is looking for an experienced Clinical Pharmacist to complement the existing team of Pharmacists and Technicians.
Working alongside our current Pharmacy Team the post-holder will quickly become an integral member of the Practice and enjoy a diverse range of duties and responsibilities including:
* Resolving medicines related queries with both healthcare professionals and patients.
* Further develop the Practice’s strong working relationships with care homes and community nursing supporting the management of patients.
* Conducting medicines/polypharmacy reviews either in a face-to-face clinical setting or by telephone.
* Developing their clinical skills in a chosen area of expertise.
* Contributing to and supporting the Audits and Quality Improvement Programme to ensure prescribing is aligned to the local formulary.
* Ensuring the achievement of targets and budgets.
* Liaising with Pharmacists working within the Leicester, Leicestershire and Rutland ICB
