Your role and responsibilities
The role is to work with your allocated customers to identify and deliver personalised, good quality outcomes to the highest of standards. You will work as part of the Home First team alongside area social work teams as well as the health and voluntary sector to support the effective integration between health and social care.
You will need
You'll need excellent communication skills to collaborate with clients, their families, advocates, and other professionals. A willingness to learn and grow is essential, as we'll help you develop your resilience under pressure and ability to stay positive. You'll be adept at taking feedback constructively and using it to improve your practice.
We will foster your gentle assertiveness, with the aim of supporting people with disabilities or injuries to regain their maximum level of skills as quickly as possible.

The team
Home First provide a short term reablement care and support service to people living in their own homes. What drives us, is to ensure people return to their normal lives in a steady manner.
As a team, we encounter fluctuations in demand in different areas, and it is important to all work together to meet demand as best as we can. It is always rewarding to see people developing their confidence and skills, and in turn, supporting newer members of staff to get into the role as quickly as they can. We are working to improve a sense of camaraderie with monthly newsletters and group workshops on specific areas of practice.
Unique to this role, is the opportunity to learn not only about reablement care, but also to gain knowledge by working jointly with the longer-term teams.
