Quest for Quality Matron
The Quest Matron will be part of a multi-disciplinary team, who will be available to support and advise staff in care homes on the prevention and proactive management of the needs of older people. The key aims of the service are to: reduce urgent admissions and attendances to hospital from care homes; reduce unnecessary reactive demand on GPs, and; provide a seamless, high quality and integrated approach to caring for people in care homes.
Main duties of the job
* Aim to minimise the effect of long term conditions on patients' lives by providing crisis intervention, stabilising conditions and preventing unnecessary A&E attendances, hospital admissions and decrease length of stay.
* Assess individual patient needs for those patients identified as high risk of admission to improve their quality of life by identifying, co coordinating and linking to appropriate care services and therapy throughout the care pathway.
* Work with the specialist teams and primary care colleagues to develop and implement a service that is NICE guidance compliant and which supports multi-disciplinary team working
* Develop patient centred goals and encourage self-management and self-care approach. The post holder will support the development of innovative evidence based practice and adopt a multi-agency approach to delivering care to care home residents
* Reduce unnecessary reactive demand on GPs,
* Provide a seamless, high quality and integrated approach to caring for people in Care Homes.
* Enhance the quality of life for people with long-term conditions
* Help people to recover from episode of ill-health or following injury
* Ensure people have a positive experience of care

Job responsibilities

* Develop and foster effective support networks locally, regionally and nationally and attend appropriate meetings relevant to role. To maintain links with all relevant agencies. Undertake on-going service review which will shape the service in response to user involvement, professional opinion and audit / survey data along with local and national guidelines/evidence.
Person Specification
QUALIFICATIONS / TRAINING
* Teaching Qualification
* Evidence of Continuing Professional Development
* Module at Masters level in at least one long term condition
* Non- medical prescriber
* Minor illness or equivalent
KNOWLEDGE, EXPERIENCE & EXPERTISE
* Substantial/significant experience of caring for patients with complex long term conditions
* Recent relevant experience in end of life care
* Community experience
* High level of presentation skills and delivery of training
* Experience of coordinating care pathways for patients with complex conditions
* Experience of multi-agency working
* Demonstrate evidence of training around treatment/support for patients with long term conditions
* Expert clinical knowledge related long term conditions and disease management
* Leadership skills and excellent decision making capabilities
* Awareness of CHFT values and vision
* Evidence of experience in Nurse led triage including telephone triage
COMMUNICATION AND RELATIONSHIPS (INCLUDING MANAGEMENT RESPONSIBILITIES)
* Ability to work independently and as part of a team
* Self-motivated and able to use own initiative
* Experience of Lone working
* Evidence of skills in assessment and management of patients with complex long term conditions and poly-pharmacy
