We're seeking a highly organized and committed Care Coordinator for our St Helens branch. This is a key role in ensuring seamless care delivery and supporting both our team and Service Users.
Key Responsibilities:
* Scheduling & Coordination Manage rotas to ensure Service Users receive consistent, quality care
* Client Assessments Support assessments and risk evaluations to develop tailored care plans
* Team Support Guide and mentor care staff to maintain excellent standards
* Stakeholder Communication Work closely with Service Users, families, and healthcare professionals
* Problem Solving Handle scheduling challenges, absences, and urgent changes effectively
* On-Call Duties Provide out-of-hours support (1 in 4 weekends and 1 weekday night)
