Frimley Park Hospital Stroke Unit are looking to recruit 2 Stroke Unit Associate Practitioners.
We are delighted to have these new positions to support us in increasing rehabilitation intensity to our stroke inpatients. You will work across the multi‑disciplinary team, assisting experienced and specialist registered staff in Physiotherapy, Occupational Therapy, Speech and Language Therapy and Dietitiacs.

Responsibilities
* Work under indirect supervision of registered therapists and clinicians to carry out rehabilitation for stroke patients, including individual or group sessions.
* Deliver rehabilitation holistically, incorporating multi‑disciplinary treatments based on each individual's rehabilitation needs post‑stroke.
* Use outcome measures, goal setting and data collection to demonstrate effectiveness of rehabilitation in line with current guidelines.
* Communicate effectively with all relevant parties, including the patient, families, and MDT members.
* Contribute to discharge planning process in liaison with MDT.
* Actively contribute to own CPD and learning, and support others in the team with their learning as appropriate.
* Work flexibly, including weekends, to deliver rehabilitation to stroke patients across 7 days.
Detailed Job Description and Main Responsibilities
* Deliver rehabilitation and treatment programmes across multiple disciplines independently, including therapeutic handling, use of medical devices and technology, exercise prescription, and group sessions.
* Recognise deterioration and improvement in patients, communicating to relevant MDT members in a timely manner and adjusting treatments accordingly.
* Use outcome measures and goal setting independently with patients.
* Have the physical ability to carry out interventions; frequent requirement to exert moderate effort during the day.
* Proactive in discharge planning, liaising with relevant parties in a timely manner and making onward referrals as appropriate.
* Participate in a 7‑day working rota.
Person Specification
Qualifications
* GCSE English and Maths or equivalent.
* Specific stroke knowledge.
Experience
* Experience as band 3 within the NHS.
* Experience delivering rehabilitation within neuro patients.
* Experience working within a team.
* Risk assessment experience.
Skills and Knowledge
* Interpersonal skills, evidence of adaptable communication with patients.
* Organise/prioritise.
* Proactive learner/reflective.
* IT skills.
