About the Role

Follow recent investment in line with Norfolk and Waveney Urgent and Emergency Care Priorities, our Urgent Community Response (UCR) Therapy Team is looking for a Band 6 Occupational Therapist. The role is based in King’s Lynn and covers West Norfolk.


Working Hours

The position operates 7 days per week between 08:00 and 20:00. Weekend and bank holiday working, as well as shift work within these hours, is required.


Key Responsibilities

* Work within the Urgent Community Response Team to prevent unnecessary admission to hospital.
* Make risk assessments and determine the safety of a patient’s ability to stay at home or return from a care setting.
* Provide evidence‑based clinical and therapeutic interventions to improve health outcomes and promote choice.
* Assess and prescribe care pathways for patients in crisis, including those with long‑term conditions, frailty, palliative or end‑of‑life care, and rehabilitation needs.
* Provide in‑reach to inpatient units and support step‑up and step‑down beds as required.
* Act as a patient advocate, facilitating choice and empowerment.
* Collaborate with health care professionals and statutory/non‑statutory agencies to deliver a seamless, integrated service.
* Triage referrals on a rotational basis for the UCR Team.
* Undertake comprehensive therapeutic assessments and develop differential diagnoses.
* Formulate and execute individualized management and treatment plans.
* Make referrals for assistive technology when appropriate.
* Communicate complex and sensitive information to patients and carers, ensuring understanding, consent, and concordance.
* Maintain therapeutic relationships and encourage patient engagement.
* Coordinate with GPs, Social Services, inpatient teams, allied health professionals, and the voluntary sector to meet identified needs.
* Participate in multidisciplinary and multi‑agency meetings, e.g. Gold Standard Framework.
* Record activity data and clinical information on SystmOne daily.
* Plan and organize specialist services within the UCR Team.
* Demonstrate skill for assessment and manual treatment, including use of specialist equipment.
* Order equipment and assist with requisitioning supplies via electronic systems.
* Obtain informed consent prior to initiating interventions.
* Adhere to universal precautions and infection control policies.


Qualifications and Experience

* Effective communication and interpersonal skills.
* A positive, problem‑solving approach to provide quality outcomes for patients.
* Resilience and flexibility in dynamic situations.
* Experience in supervising staff and students is desirable.


Supervision and Development

Clinical supervision is provided by a Band 7 Clinical Lead Therapist. The team supports professional growth and offers opportunities for ongoing development.
