Main duties of the job In this role, you will Support the Partners in implementing effectivemedicine management and patient care within the practice/s. Job description Job responsibilities Deliver patient-facingclinical care: Conduct consultations, prescribe medication,run clinics (especially for long-term conditions), and support vulnerable orcomplex patients, including home visits. Enhance patientunderstanding and outcomes: Educate patients and carers on medicationuse, improve adherence, and provide ongoing support to build confidence intreatments. Ensure safety and qualityof prescribing: Monitor high-risk drugs, handle medicationincidents, carry out audits, and align practice with clinical standards (e.g.,NICE) to improve safety and performance. Collaborate, train, anddevelop services: Work within the multidisciplinary team,support staff training, liaise with external stakeholders (e.g., communitypharmacists), and contribute to service improvement and professionaldevelopment. Enhance patient understanding and outcomes: Educate patients and carers on medication use, improve adherence, and provide ongoing support to build confidence in treatments. A working understanding of NHS policies, processes, structures, systems and clinical practices. Knowledge of the Community Pharmacy Contract.
